1. CAN I JUST PRINT OFF ALL OF THE FORMS? CAN YOU MAIL THEM TO ME?
You will be required to submit an application online however if you are having difficulties with the form or do not have a computer, our staff can assist you. The medical record release form cannot be completed by our staff but can be mailed to you if necessary.
2. ARE THERE TIME LIMITS FOR HOW OLD THE RECORDS COULD BE?
This is condition dependent.
Generally conditions which should have healed over time will not qualify a patient if the records show the last care received being greater than a year ago (Strains, Sprains, ER, Zoomcare).
We will review all records submitted for qualifications and do our very best to issue a recommendation as to what would be required to qualify for a recommendation.
1. CAN I PRINT OFF THE PATIENT INTAKE FORM AND BRING IT TO MY APPOINTMENT?

Minors under 18 are not allowed inside any part of a dispensary where marijuana is present, even if they are patients or employees.

1. AS A GROWER, WHAT IS THE MAXIMUM AMOUNT OF PLANTS I MAY GROW?
As a patient or registered grower, you can grow
- twelve (12) plants under 1’ x 1’ and,
- six (6) plants of any size
- These plants will need to be grown at a registered growth site.
1. DOES MEASURE 91 MAKE CHANGES TO THE OREGON MEDICAL MARIJUANA PROGRAM?
No.
Measure 91 specifically exempts the current Oregon Medical Marijuana Program from its provisions.
1. ARE THERE ANY LEGAL CONCERNS WHEN I CHANGE MY MAILING ADDRESS, ADD OR REMOVE A CAREGIVER, GROWER, OR CHANGE MY GROW SITE ADDRESS?
Yes.
It is important that you have the correct information in your file so that if you are contacted by law enforcement and they request verification of information from the OMMP, the caregiver, grower and grow site are all protected. It is equally important that the former caregiver and/or grower know of their status so they are aware they are no longer protected.
In the case of a grow site change, remember that a grow site registration card must be posted at the registered grow site. If you change your grow site and do not yet have a grow site registration card posted at the new grow site, you may not be protected from criminal penalties. The OMMP recommends that you obtain a new grow site registration card before moving plants to a new grow site and keep copies of all change forms.
*Note: To make sure all appropriate individuals and the grow site are protected and that previously protected individuals know their status has changed, it is best to make all changes (notify OMMP in writing, notify former caregiver and/or grower, and move grow site) on the same day or as quickly as possible
